Objective:To evaluate the efficacy and safety of a generic, low cost, unbranded oral antiobesity drug (Orlistat, Liraglutide, GSK, and Novo Nordisk) in reducing cardiovascular risk in patients with coronary heart disease (CHD) who have previously had an MI and were at a higher risk of developing CVD and stroke. Materials and methods: We conducted a prospective, randomized, double-blind, placebo-controlled study in 56 patients with CHD with a baseline MI diagnosis of within 3 months of enrollment. All patients were randomly assigned to Orlistat, Liraglutide, or Novo Nordisk. Patients with MI who had previously been at a higher risk of CVD or stroke were excluded from the study. We evaluated whether Orlistat (N = 57) had a reduced cardiovascular risk; the other two drugs were not studied. We also evaluated whether there was an increased cardiovascular risk with the other two drugs. We conducted an analysis of all adverse events occurring during the 6-month follow-up period. Patients were followed for a mean of 18.6 months (range: 1 to 36 months) and all patients were monitored for changes in vital signs. Risk ratio for the reduction in CVD events was estimated using logistic regression. All patients were also monitored for a change in stroke event rates (including stroke-related death and stroke/TIA) and cardiovascular event rates (including heart attack and death) and for the occurrence of any CVD events (including cardiovascular events occurring in patients with a history of CVD or stroke). A secondary analysis was performed using Cox proportional hazard models to assess whether the changes in CVD events were a significant risk factor for stroke. Results: The incidence of stroke, TIA, heart attack, stroke/TIA events and cardiovascular events were similar in the two groups. However, there was a greater risk of stroke in patients with a history of CVD compared with those without a history of CVD (adjusted odds ratio [AOR] 0.83, 95% confidence interval [CI] 0.60 to 1.14). The overall rate of stroke was reduced with the use of Orlistat and the other drugs (AOR 1.02, 95% CI 0.94 to 1.12). There were no differences in the rate of any CVD events in the two groups. The relative risk of heart attack and stroke was 0.80 and 0.79, respectively. Conclusions: Orlistat (N = 57) had a reduced cardiovascular risk relative to the other drugs (AOR 0.84, 95% CI 0.60 to 1.14). Our results suggest that Orlistat (N = 57) can be used in patients with a prior MI and is an effective medication for reducing the risk of CVD and stroke. As such, it should not be used in patients with a history of CVD or stroke.
